Abstract

This study aimed to report the endodontic management of a mandibular first premolar with three canals and independent foramina. In most cases, premolars present a single and broad root canal. However, mandibular first premolars may present variations in morphology and in number of root canals, which increases the endodontic treatment complexity. The tooth presented a trifurcation in the middle third and three distinct canals with independent foramina. Endodontic treatment was performed with the aid of an operating microscope. Cone beam computed tomography and operative microscope were essential to establish the correct diagnosis given the mandibular premolar anatomic variation. Thus, the association of these two tools facilitated a proper endodontic management, increasing the chances of success.
